WAIVER

TEK RMD DEMO RELEASE AND WAIVER OF LIABILITY AGREEMENT

I understand that Matia Robotics is not warranting that the Tek RMD is fit for my use or for any particular person, and that I am solely responsible for demonstrating the Tek RMD.

I hereby acknowledge that:

  • Use of the Tek RMD requires a physician’s written prescription
  • My body weight is minimum 88.1lbs (40Kg) and maximum 264.5lbs (120Kg)
  • My body height is minimum 4.6ft (140cm) and maximum 6.2ft (190cm)
  • My difference in leg length is less than 2in (5cm)
  • I do not have restrictions in hand function that interfere with operation or control of the Tek RMD
  • I do not have restrictions in force or range of motion in the shoulders or upper extremities that interfere with autonomous verticalization in the Tek RMD
  • I do not have severe contractures at the hip, knee or ankle joint(s) that prevent verticalization in the Tek RMD (>30% of normal range of movement)
  • I do not have osteoporosis in my lower legs or spine
  • I do not have autonomic dysreflexia, hypotension and/or dizziness
  • I do not have a history of epilepsy
  • I do not have cognitive restrictions that interfere with the safe use of the Tek RMD
  • I am not pregnant)
  • I am not visually impaired (blind)

I HEREBY RELEASE FROM LIABILITY AND AGREE TO INDEMNIFY AND HOLD HARMLESS MATIA ROBOTICS, AND ANY OF IT’S EMPLOYEES, AGENTS OR VOLUNTEERS REPRESENTING OR RELATED TO MATIA ROBOTICS, FOR ANY LIABILITY IN CONNECTION WITH THE TRIAL OR THE USE OF THE TEK RMD. THIS RELEASE IS FOR ANY AND ALL LIABILITY FOR PERSONAL INJURIES (INCLUDING DEATH) AND PROPERTY LOSSES OR DAMAGE OCCASIONED BY, OR IN CONNECTION WITH THE POSSESSION OR USE OF THE TEK RMD.

I HAVE READ AND UNDERSTOOD THE FOREGOING TEK RMD DEMO RELEASE AND WAIVER OF LIABILITY AGREEMENT.
